Role of Biliary Drainage Procedures for Intrahepatic Stone




Abstract
Biliary drainage procudures for the patients with intrahepatic stones are aimed to eliminate bile stasis and permit residual stones to enter the intestinal tract. However, problems such as ascending cholangitis, failure of spontaneous passage of
stones
or procudural complications are considered as potential disadvantages of the procedures. The operative results in a consecutive series of 211 patients who underwent biliarydrainage procedures(n=96) and no biliary drainage procedures(n=115) over a
period
fo 14 years were reviewed. The residual stones for biliary drainage were 23 of 96 patients (24.2 percent) and 28 of 115 patients(24.3 percent) for no biliarydrainage. Follow up study showed good results in 77.1 percent of patients with biliary
drainage
and 82.6 percent with no biliary drainage which was statistically significant. The recurrent stones were observed in five of 67 followed up patients(7.5 percent) with biliary drainage and four of 78 patients(5.1 percent) with no biliary drainage.
This
was not a statistically significant difference, but follow up study of the ptients with recurrent stones revealed four of the five patients in the biliary drainage group with severe cholangitis and all of the four patients in the no drainage
group
were
in good health. Authors conclude that biliary drainage procedures should be reserved for instances with proper indication.
